Skeletal manifestations of hypoparathyroidism mimicking ankylosing spondylitis; a rare clinical presentation: A case report

Gunatilake Sonali , Bulugahapitiya Uditha

Introduction: Idiopathic hypoparathyroidism is a rare endocrine disorder with varying clinical manifestations ranging from acute hypocalcemic symptoms to asymptomatic soft and skeletal tissue calcifications/ossifications. Patients may present with wide spectrum of rheumatological manifestations, which are under-recognized, thus delaying timely treatment. This results in chronic disability.Case presentation: A 47-year-old male with past history of convuls...

A rare case of a malignant prolactinoma presenting with skull metastasis after two years of a masked diagnosis

Abeyaratne Dayakshi , Gunatilake Sonali , Joseph Naveen , Korbonits Marta , Somasundaram Noel

Background: Pituitary carcinoma is extremely rare and constitute only 0.1−0.2% of all pituitary tumors. Diagnosis is on evidence of metastasis, although these criteria has been challenged. Majority of pituitary carcinomas are functioning tumours, usually secreting ACTH (42%) or prolactin (33%). Common sites of metastasis include the brain, spinal cord, leptomeninges, bone, liver, lymph nodes and lung. Mean survival after detection of metastasis is around 1−2 years....

Short term metabolic outcome following bariatric surgery; A single center experience from Asia

Gunatilake Sonali , Weerakoon Diana , Subasinghe Chandrika , Bulugahapitya Uditha , Wijerathna Thejana

Background: Obesity and related co-morbidities have become an emerging major health concern in South Asia. In addition to the efficacy in weight control, bariatric surgery improves metabolic disorders, thus is a recognized mode of treatment in obesity. Objectives: To review the outcome of metabolic disorders in a group of obese patients in Sri Lanka following bariatric surgery. Methods: Patients who underwent bariatric surgery at Pro...

Recovery of the hypothalamic-pituitary-adrenal and thyroid axes up to 12 months following trans-sphenoidal adenomectomy

Gunatilake Sonali , Pofi Riccardo , Macgregor Victoria , Cudlip Simon , Jafar-Mohammadi Bahram , Tomlinson Jeremy , Pal Aparna

Background: Hypopituitarism is a potential complication of trans-sphenoidal adenomectomy (TSA). Recovery of pituitary function can occur, and reassessment is required to avoid unnecessary hormonal replacement. However, frequency of re-testing is variable across centres. Aim of this study was to determine recovery rates and time to recovery of hypothalamo-pituitary adrenal (HPA) and thyroid axis after TSA.Methods: We performed a single-centre, retrospecti...

A rare presentation of ovotesticular disorders

M Ratnayake Gowri , Chandana UDS , Chandrasiri Upali , Abeysinghe Chandima , Dassanayake Wajira , Maddumabandara HRL , Liyanage PPS , Ruwanpura Lahiru , Gunatilake Sonali

Introduction: The prevalence of gynaecomastia ranges between 40-60% and the majority of cases are asymptomatic, commoner among obese and is due to excess circulating oestrogen. Ovo-testicular disorders of sex development (OT-DSD) is one of the rarest disorders of DSDs and commonly presents with bilateral intra-abdominal gonads and ambiguous external genitalia.Case presentation: A 15 years old male presented with progressively worsening bilateral gynaecom...

Frequency and timing of hypopituitarism as a consequence of pituitary directed radiotherapy; a retrospective cohort study

Goindoo Ryan , Hussein Furhana , Surchi Haval , Gunatilake Sonali , Ortez-Toro Jose , Cudlip Simon , Hobbs Claire , Jafar-Mohammadi Bahram , Pal Aparna

Background: Patients receiving radiotherapy for residual or recurrent pituitary adenoma require regular surveillance for the development of anterior pituitary axis deficit. Whilst the sequelae of hypopituitarism post cranial irradiation is well recognized, there are relatively varied incidences of new onset hypopituitarism post-conventional radiotherapy, reported in the literature. We aimed to investigate timing and frequency of the individual axis deficits in adults who have ...

Recovery of the hypothalamic-pituitary-adrenal and gonadal axes following trans-sphenoidal adenomectomy, a single centre experience

Pofi Riccardo , Gunatilake Sonali , Macgregor Victoria , Cudlip Simon , Joseph Robin , Lenzi Andrea , Tomlinson Jeremy W , Jafar-Mohammadi Bahram , Isidori Andrea M , Pal Aparna

Hypopituitarism is a potential complication of trans-sphenoidal adenomectomy (TSA). Prediction of pituitary function (PF) recovery would inform hormonal replacement strategies. However, the frequency of re-testing of PF is variable across centres. The aim of this study was to determine rates, time and predictors of 6-weeks (6 w) recovery of hypothalamo-pituitary adrenal (HPA) and gonadal function after TSA.Methods: We performed a single-centre, retrospec...

A model to predict Hypothalamic-Pituitary-Adrenal (HPA) axis recovery at 6 weeks post trans sphenoidal adenomectomy: a single-centre retrospective analysis

Pofi Riccardo , Gunatilake Sonali , Isidori Andrea M , Lenzi Andrea , Macgregor Victoria , Cudlip Simon , Jafar-Mohammadi Bahram , Tomlinson Jeremy W , Pal Aparna

Background: Hypopituitarism can occur post trans-sphenoidal adenomectomy(TSA). Accurate prediction of HPA axis recovery would inform hydrocortisone replacement strategies but there are limited studies on this. In our centre, HPA axis is assessed with pre-operative short synacthen test (SST), day-8 post TSA 9am cortisol and 6-week post-TSA SST. Patients are commenced on hydrocortisone post TSA until 6 weeks review. Methods: We performed a single-centre, lo...
